Language Placement Reporting Form
This form is to be used by language coordinators and placement contacts to report a placement determined by consultation or a change in placement.
Leave of Absence Form
In advance of your leave of absence appointment, you may fill out this form to bring with you. Learn more about our policies on the Leave of Absence section of this site.
Readmission Form
Students who have been withdrawn from the College are required to submit a completed readmission application along with supporting materials eight weeks prior to the start of the quarter of intended return. The application will be reviewed by the Faculty Committee on College Re-admissions. The results of the review will be communicated at least four weeks in advance of the intended quarter of return. The College is not obliged to approve student re-admissions. Upon return, students are expected to complete the requirements for the degree without further interruption. General Petition
Use this form to request exemption or special consideration from various College rules and requirements.
Non-Law School Student Permission to Register
Petition to take Physical Sciences 10900-11000, 13500-13400, or 13600
If you wish to enroll in these courses, attend the first scheduled lecture. During the first session, you will be given a special petition to complete and return. We will notify you the results by email, usually during the first week of classes.
Language Petition
This form is for students who have lived and gone to school in another culture and wish to document that for their language competency requirement, and for students who wish to get credit for language courses in the second year and beyond after taking a course they placed into.
